USA: Doctor’s licence revoked for unprofessional conduct

Vermont Board of Medical Practice on August 2, 2010 ordered that Dr. William A O’Rourke has been found guilty for unprofessional conduct & prescribing a controlled substance on two occassions without examining or taking the medical history of the patient. Dr. O’Rourke’s licence (Licence No.: 042-0002399) has been revoked.

Australia: Nurse Lynda Gran reprimanded

On August 26, 2010 the Health Care Complaints Commission prosecuted Ms Lynda Grant, a registered nurse and midwife, before the Nurses and Midwives Tribunal. The Commission alleged that Ms Grant directed another nurse to disconnect a patient’s bedside buzzer and failed to reconnect the buzzer when she attended to the patient. The Commission also alleged that Ms Grant made inadequate entries in the patient’s medical notes.

USA: Doctor pleaded guilty of medical malpractice

On July 7, 2010 the Board of Medicine approved a Settlement Agreement for Robert H. Richardson, M.D. Dr. Richardson’s license lapsed on June 30, 2007. He continued to practice medicine and prescribe controlled substances and other medication for patients after his license lapsed. Dr. Richardson is reprimanded and is required to participate in CPEP for an evaluation of his clinical skills and his mental and physical ability to practice medicine.

USA: Doctor reprimanded by the New Hampshire Board of Medicine

On July 7, 2010 the Board of Medicine approved a Settlement Agreement for Bruce D. Latham, D.O. The Board received information pertaining to the quality of care provided to some of his patients. Dr. Latham failed to adequately document the rationale underlying his decision-making related to diagnostic testing and therapeutic treatment on four patients. Dr. Latham is reprimanded; he has participated in an 8-hour Center for Personalized Education for Physicians (“CPEP”) Patient Care Documentation Seminar. Dr. Latham is required to participate in an assessment of his medical skills at the University of New England, College of Osteopathic Medicine and provide the Board with documentation of completion of any recommendations made by the assessment.

Nasif, M.D. Before the Board are professional misconduct allegations concerning pending disciplinary charges. Dr. Nasif relinquishes all rights and privileges to practice medicine in the State of New Hampshire.

On July 7, 2010 the Board approved a Preliminary Agreement for Practice Restrictions for Susan M. Hare, M.D. Dr. Hare has professional misconduct allegations pending before the Board. Dr. Hare voluntarily agrees not to write prescriptions in the State of New Hampshire for Schedule II and/or III narcotics until the conclusion of adjudicatory proceedings arising out of the allegations.

USA: Dr. Kevin R. Costin reprimanded and fined

On July 7, 2010 the Board of Medicine approved a Settlement Agreement for Kevin Costin, P.A. Mr. Costin wrote six prescriptions for an individual, without establishing a valid physician-patient relationship with her, without the authorization of the individual’s treating physician or his own responsible supervising physician, and without noting five of those prescriptions in the individual’s medical chart. Mr. Costin is reprimanded and is required to complete (15) hours in the area of professional boundaries and documentation. He is assessed an administrative fine in the amount of $3,000.

Dr. Calegari sold prescription-strength medications through a website without a valid physician-patient relationship. Dr. Calegari is reprimanded and assessed an administrative fine in the amount of $1,000.

Mazur, M.D. The Board has received information indicating that the continued practice of medicine by Dr. Mazur presently poses an imminent threat to life, safety and/or health, which warrants the temporary suspension of Dr. Mazur’s license to practice medicine pending a hearing that is scheduled for July 7, 2010.

My Eagle News on August 25, 2010 reported that Lindsay P. Madden, a nurse, has been pleaded guilty to seven drug-related counts including two felonies – tampering with drug records and possession of a controlled substance.

United Kingdom: Doctor Pleaded Guilty for fake blood scandal

The Independent on August 26, 2010 reported that Dr. Wendy Chapmon had been pleaded guilty in the fake blood scandal that cost Dean Richards his job as director of rugby. Dr. Chapmon has admitted cutting the lip of the Quins wing Tom Williams, who had bitten on a blood capsule as part of a substitution scam and was attempting to conceal the deceit.
