Crews III, 41, embezzled about $17,000 from the program by submitting invoices for the purchase of handicapped-accessible vans at inflated amounts. He supplied two vans to the program but received payments from the program for five. He will have to forfeit $64,000. The scheme was aided by a former program claims manager, Iris Allen, who is now serving 10 years in federal prison for embezzling nearly $1 million from the program.

Stegawski, who received a medical degree in Poland in 1977, represented himself as a chronic pain management doctor at the Dayton and Lucasville clinic and an unnamed clinic in Southpoint.

The doctor is 56-year-old Peter Hickox, who worked at Lodi memorial and P. Gill Medical Group, until a janitor uncovered his potentially dangerous secret. More than a year ago, Hickox was allegedly hooked on what are known as poppers, chemicals used to clean video cassette players.

During this time, Tran may practice only when he is monitored by a physician who is board certified in family medicine. The penalties take effect May 24, according to the Board for Professional Misconduct. Tran has been licensed to practice in New York since July 2001. He will continue to practice medicine during the probation, according to a statement from his attorney, Richard S. Tubiolo.

Gant had worked at the center about a month before the accusation, according to court records. Court documents say Gant failed to inform medical personnel of the deteriorating condition of patient Mario Martinez, who died Oct. 20, 2009.
